The previously stated plan is for Cole to "de-load" after a few weeks before meeting with Dr. Neal ElAttrache this offseason. Manager Aaron Boone said he's hoping to see Cole "face live hitters during camp." The 35-year-old former Cy Young winner and six-time All-Star is making progress from Tommy John surgery on his right elbow, but he's not expected to be ready for the start of the 2026 regular season. The good news, though, is that Cole isn't expected to be out long early in the season before making his debut. Fantasy managers are going to have questions as to whether Cole can regain his pre-injury form at his age after making only 17 starts in 2024 and not pitching at all in 2025.

Cole made only 17 starts for the Yankees in 2024 due to injuries and was unable to pitch at all in 2025, so it's understandable for fantasy managers to wonder if he can return to his Cy Young form in 2026.

New York Yankees right-hander Gerrit Cole (elbow), who is rehabbing from Tommy John surgery on his right elbow, is scheduled to throw 45 pitches from 75 feet on Wednesday, according to Jack Curry of the YES Network. "Feels great," Cole said. The 35-year-old veteran is coming along nicely after starting a throwing program last month following TJ surgery and an internal-brace procedure back in March. He will continue to rehab throughout the offseason, but it remains to be seen if he will be cleared to rejoin New York's starting rotation for Opening Day in 2026. The six-time All-Star and Cy Young winner only made 17 starts due to injury in 2024 and did not pitch at all for the Yankees in 2025. While Cole still has ace upside when healthy, his injuries over the last two years will make him more of a risk/reward SP1 in fantasy drafts.

It's a big milestone for Cole, who had reconstructive surgery on his right UCL back in March. The 34-year-old veteran will still miss the entire 2025 season, and it's not guaranteed that he'll be ready for Opening Day in 2026. Cole was dominant with a 2.63 ERA, 0.98 WHIP and 222 strikeouts in 33 starts for the Yankees in 2023 as the American League Cy Young winner, but this is his second straight injured season after he made only 17 starts in 2024. He will still be relevant in all fantasy leagues next year as a high-end starter, but it's fair to wonder now whether Cole won't be as dominant as he nears the back nine of his MLB career.

There is no target date in mind for Cole to pitch next season, and we probably won't have a better idea of that until he resumes throwing off the mound, which might not come at all this year. The six-time All-Star won the American League Cy Young award in 2023 but then only made it through 17 starts (95 innings) for the Yankees last year, going 8-5 with a 3.41 ERA, 1.13 WHIP and 99:29 K:BB. There's no guarantee that Cole will be ready for the start of the 2026 season, which will put a cloud over his fantasy value.

Cole will still miss the entire 2025 season, but the internal-brace procedure means that he could have a chance to be ready right around the start of the 2026 season, depending on how he fares during his rehab. Fantasy managers that already drafted the 34-year-old former American League Cy Young winner in single-year leagues can dump him now that he's officially out for the year. The Yankees are also going to be without starter Luis Gil (lat) to begin the year, which means that both veteran Marcus Stroman and pitching prospect Will Warren figure to open the year in the starting rotation.

New York Yankees starting pitcher Gerrit Cole (elbow) is set to undergo Tommy John surgery on Tuesday to repair a torn right ulnar collateral ligament. He will miss the entire 2025 season. The 34-year-old underwent testing on his right elbow over the weekend. He went for a second opinion earlier on Monday. In addition to missing the entire 2025 campaign, the 34-year-old could also be in danger of missing a significant portion of the 2026 season as well. Last season, Cole missed time due to elbow inflammation but was able to log 95 innings with a 3.41 ERA and a 1.13 WHIP. He tallied 99 punchouts and held a 7.4 percent walk rate. He generated a modest 3.61 xERA with a .226 xBA. In addition to Cole, right-handed pitcher Luis Gil (lat) is set to miss at least three months of action. This will open the door for Marcus Stroman to have a permanent spot in the rotation and potentially prospect Will Warren to open the season as the No. 5 option.

While nothing is official yet, the 34-year-old former American League Cy Young winner should be avoided in fantasy formats with the distinct possibility that he won't be pitching at all in 2025. Even if Cole likes what ElAttrache says on Monday and he decides to go with a non-surgical route, he is going to be out for the foreseeable future and will probably require TJ surgery eventually. We'll have more information on Cole when it becomes available, but things are looking grim for the Yankees' ace right now.

Gerrit Cole Recommended For Tommy John Surgery, Awaiting Second Opinion

The ominous "concern" surrounding New York Yankees starting pitcher Gerrit Cole's throwing elbow received some devastating clarity on Sunday when it was reported that the veteran has been recommended to undergo Tommy John surgery. The six-time All-Star is still awaiting a second opinion, but GM Brian Cashman had already stated they were preparing for the worst. It would be a big blow to the Yankees' hopes of making another World Series run and for any fantasy managers who already drafted the former Cy Young winner. When one door closes, another one opens, though, and New York's fifth-overall prospect Will Warren could nab a rotation spot in the absence of Cole. Warren has been impressive this spring, allowing just one earned run on two hits and two walks while striking out 11 in eight innings of work. Keep an eye out for the results of Cole's second opinion, but for now, it looks like he should be avoided in drafts, while Warren could be a worthwhile flier in later rounds.
