Jose Berrios Hit Hard in First Rehab Game

Toronto Blue Jays right-hander Jose Berrios (elbow), who is attempting to pitch through a minor stress fracture in his right elbow, was hit hard in his first minor-league rehab start on Thursday at Single-A Dunedin. Berrios allowed five runs on five hits while walking one and striking out three in 2 2/3 innings. He's a bit more stretched out than other pitchers when they make their first rehab start, and he was able to get up to 47 total pitches in the outing. The results weren't pretty, but Berrios averaged 94.2 mph with his fastball, which was up 1.2 mph from last year. His injury sounds serious, but he's been cleared by doctors to pitch through it, and he hasn't reported any symptoms. Right-handed pitching prospect Trey Yesavage (shoulder) will likely return from the 15-day injured list (possibly next week) before Berrios does. Berrios could return to the big leagues in the coming weeks, depending on whether he needs one, two, or three rehab appearances. When he does return, the 31-year-old Puerto Rican veteran could become a long man out of the bullpen, at least initially.

Berrios has been an innings-eater for fantasy managers, but he's much riskier this year with declining velocity and obvious injury concerns. Once Trey Yesavage (shoulder) and Shane Bieber (elbow, forearm) are ready to return to the rotation, too, it's possible that Berrios could be moved to a long-relief role if he's struggling as a starter for the Jays upon his return from his elbow injury.

Berrios is on the 15-day injured list to begin the 2026 regular season after being diagnosed with a stress fracture in his right elbow last week. The Blue Jays are hoping that the 31-year-old can continue to pitch through it, but the problem is that he hasn't appeared in a spring training game in three weeks, so he needs to build his arm back up. The veteran Puerto Rican doesn't have a timetable for when he might make his season debut in Toronto, but barring a setback, it could be possible for him to rejoin the Blue Jays before the end of April. Berrios went 9-5 in 2025 in his fourth full season with the Jays, recording a 4.17 ERA, 1.30 WHIP, and 138:56 K:BB in 166 innings pitched. Since the start of the 2019 season, Berrios has more innings on his arm than any other pitcher, and all the tread on his tires appears to be catching up with him. When the 31-year-old is ready to rejoin the big-league squad, it could come in a long-relief role, which would kill most of what's left of his fantasy value. Berrios is no longer as dominant as he once was, he has injury concerns, and his home run issues probably aren't going away at hitter-friendly Rogers Centre.

While the Blue Jays hope he can continue pitching through his elbow injury, it wouldn't be wise to bank on it and trust him in fantasy baseball leagues. When Toronto's entire rotation is healthy, Berrios might be ticketed for a relief role in 2026.

While it doesn't seem too serious right now, fantasy managers must remember that Berrios also had some similar elbow and biceps issues late last season. The former first-rounder has a lot of tread on his tires, having thrown 1,571 2/3 innings in his 10-year big-league career. With some injury issues this spring to boot, Berrios is not in RotoBaller's top-100 starting pitchers going into the 2026 season.
